Enhance Physiotherapy – Terms & Conditions of Service

Effective from August 2025

Introduction

These Terms & Conditions apply to all patients of Enhance Physiotherapy. By booking or attending an appointment, you agree to these Terms & Conditions.

 

Scope of Services

We provide physiotherapy and related health services, which may include assessment, diagnosis, treatment, rehabilitation, education, and prevention advice.
We do not provide emergency medical services. In the case of a medical emergency, please call 000 or attend your nearest emergency department.

 

Your Responsibilities

You agree to:

  • Provide accurate and complete personal and medical information.
  • Inform us promptly of any changes to your health, medications, or personal details.
  • Follow the treatment plan and home exercise program as discussed with your physiotherapist.
  • Treat our staff and other patients with respect.
  • Attend appointments on time.

 

Appointments & Cancellations

  • Please arrive on time for your appointment. If you arrive late, your session may be shortened so we can see the next patient on time.
  • We require at least 4 hours’ notice if you need to cancel or reschedule an appointment.
    • Late cancellations or non-attendance may incur a cancellation fee equal to half of the appointment cost.
  • We may waive this fee at our discretion in exceptional circumstances.

Fees & Payments

  • Fees are payable in full on the day of your appointment, unless otherwise agreed.
  • We accept [list payment methods, e.g., EFTPOS, credit card, cash, HICAPS].
  • If you have private health insurance, you may be able to claim part of the fee via HICAPS at the time of your appointment.
  • Medicare, DVA, NDIS, or other funding body claims will be processed in accordance with their rules and requirements.

 

Health Records & Privacy

We keep health records in line with our legal and professional obligations.
Our Privacy Policy explains how we collect, use, store, and protect your personal information, including your rights to access and correct it.
You can view our Privacy Policy on our website or request a copy from reception.

 

Informed Consent

Physiotherapy treatment may involve physical examination, manual therapy, exercise prescription, and the use of clinical equipment.
You will be informed of the nature, purpose, and any known risks of the proposed treatment before it begins.
By proceeding with treatment, you give your informed consent. You can withdraw your consent at any time.

 

Limitation of Liability

We will provide services with reasonable care and skill. However, outcomes depend on many factors, including your personal commitment to the treatment plan.
We cannot guarantee specific results, and to the maximum extent permitted by law, we are not liable for any loss or damage arising from the services we provide, except as required by Australian Consumer Law.

 

Telehealth Services

If you receive services by telehealth:

  • We will explain how the consultation will be conducted.
  • You are responsible for ensuring you are in a private, safe space.
  • You accept the risks of technology failures or interruptions.
  • Some conditions may require in-person assessment for safe and effective care.

 

Communication

We may contact you by phone, SMS, email, or post for appointment reminders, treatment updates, or administrative matters.
While reminders are a courtesy service, it is your responsibility to attend your booked appointments.

 

Changes to These Terms

We may update these Terms & Conditions from time to time. The current version will always be available on our website and in our clinic.

 

Governing Law

These Terms & Conditions are governed by the laws of Western Australia.

By attending an appointment or engaging our services, you acknowledge that you have read, understood, and agree to these Terms & Conditions.
